We are looking to immediately hire a Board-Certified Nurse Practitioner to provide medication- assisted treatment (MAT) and some primary care to individuals with substance use disorders at our inpatient hospital in Coburg, OR and occasionally travel our outpatient office in Eugene, OR.
Collaborating physician relationship provided.
Full-time Monday-Friday SCHEDULE:
Monday - Friday; day shift MAIN DUTIES:
Provide medical services to patients including health assessment, medication evaluation and management, and ongoing support for patients with substance use and co-occurring mental health disorders Complete full and appropriate documentation to support professional billing compliance Provide education regarding illness and prognosis to assist with clarification of patient goals of care and support development of a comprehensive integrated care plan Interpret test results to identify any health issues and provide feedback to patients Educate and counsel patients regarding medication compliance Work with an interdisciplinary team of professionals to provide integrated care for our patients.
Review and oversee progress and care plans, including making any adjustments Maintain required licensure, certifications, and CME credits
Qualifications:
Master of Science degree with completion of an accredited Nurse Practitioner Program required.
Current state licensure as a Nurse Practitioner required.
Current state licensure as a Registered Nurse required.
Current DEA licensure and prescriptive authority in Oregon, required.
Min.
