Social Worker (MSW) - Home Care
As a Social Worker (MSW) - Home Care you will be responsible for serving as a liaison between the client and their family and agency staff and the community to assist in the planning of long-term care for each client.
Additional responsibilities of the Social Worker (MSW) - Home Care include:
Assessing client need for community referrals.
Providing or arranging for social services for clients and family members as appropriate.
Completing all documentation related to social work.
Educating families and clients regarding placement, care, and rights.
QUALIFICATIONS :
Social Worker (MSW) - Home Care
To qualify for the Social Worker (MSW) - Home Care, you must have effective communication skills and be able to work with individuals of all ages.
Additional requirements of the Social Worker (MSW) - Home Care include:
Master's degree in Social Work or a human services field.
Licensure or certification as a social worker in the state where the agency is located, if required by State Social Work licensing board.
One year of social work experience in a health care setting by definition in the Medicare Conditions of Participation.
Must be at least 18 years of age.
Must have a favorable motor vehicle record (MVR) and must present a valid drivers license and proof of insurance before operating a vehicle.
